**Changes due to COVID-19**
All requests need to be made via email and requests may take longer than 3 days due to the current situation. Once you have emailed your request form, please mail a check for the records you have requested based on the fees listed below.

The Records Section receives and maintains all records produced by the Police Department. This includes incident reports, accidents reports, arrest reports and traffic summonses, etc.

Traffic Accident Report

To obtain a copy of a Police Report, please fill out the Request for Records form.

You can email the form to supportservices@fountainpd.com, fax it (719) 382-7790
or drop it off at the Police Station, located at 222 N Santa Fe Avenue. Once the report has been completed, and if the report is able to be released, it will be released within 3 business days.
